The online Self-Compliance Reporting tool is meant to assist property owners who have received a pre-citation or citation and wish to report compliance.  If you have already submitted a self-compliance report for an open case, do NOT start a new report.  Please refer to the confirmation email you received from your initial submission.  

Please follow the instructions listed below.  

Instructions

1. Enter Property Address
2. Enter Inspection/Case Number.  The case/inspection number is not required to submit the report.
3. Enter a description in the Describe Your Problem or question in detail box or state “Reporting Self Compliance.”
4. Click on the "Drag image(s) here, or click to select file(s)" to upload your photos.  You can only upload a maximum of 5 photos. 
5. Press SUBMIT
6. Enter your Contact Information
7. You will receive a confirmation e-mail confirming receipt.
